Itch.io should add some moderation on these comments, ik someone wants to say their opinion but these kids getting easily offended by NTR is just insanity and it even ruins the game overall, someone wants to enjoy a game and then sees this, it makes people lose taste to play at all

"real" men are quite the pussies, if they need to affirm their manliness by leaving a bad review for a game they did not even play. It is a thing woke activists would do. A form of virtue signalling.

Bad ratings are not fake because they are bad. Your ratings look like an expected value. The game is not mainstream and your images are very vulgar and intense. And it has some things that are not everyone's cup of tea. Be it the threesome, the watersports, the preganancy or whatever.
So you have few ratings and comments on the one hand and some hater ratings on the other.
But the game is obviously popular if you look at other metrics, like your follower count here and on patreon.

And that tiny 3 liner description is even misleading! Your game is not a https://itch.io/games/tag-side-scroller and I have doubts, that is an https://itch.io/games/tag-open-world game. It was a long time since I tried it. It was not open world back then. It is also not really a visual novel. Not in my opinion.
From my observation, there are people that are surprised by the "gay" content. It is not gay, of course. But to them, it is. To them it is just as bad as a surprise trap character undressing. This gets people emotional. And emotional people give emotional ratings. Without the engagement, people just pass and move on to other games. Why waste time to downrate a game they would not want to play. But if they did take the effort to download your very big files only to find out, that the game features content, that they hate ... well, that's what's happening in my opinion. At least part of it.
It does not help that you included pregancy content. A big turn off for some. I for one want to play a light hearted lewd game placed in a magical world, where people can have anal sex without preparation and do not have to worry about std or menstruation or birth control. It breaks immersion, unless your game is about "breeding" as a core mechanic. Characters inside the game would know about the risks of intercourse and take steps. Like condoms and birth control.
But the hypothesis is this: the type of player wanting to have a harem of girls that he can even get pregnant is very likely also the type of player that would very much hate it, to see any of those girls near a dick that is not the dick of the main character.

What does free version mean? Not sure if here for every game that could mean something slightly different, like a better version never appearing here and stuff like that and only ever on patreon.
free versions are usually 1 or 2 month old versions of the game
latest versions are paid and they will be available on patreon
